Transportation Agreement:

• Download and complete Transportation Agreement.
• Fax it to SPI head office (866-383-0393), prior to loading your first load.
• Remember to attach copies of your authorities and insurance for our files.
• Have your insurance company mail an original insurance certificate to SPI naming Silver Pacific Investments as the certificate holder.

*Please note, we cannot dispatch any freight to your company until we receive this documentation and you are set up in our computer system.

Dispatch Agreement:

• You will receive a written load confirmation for each load you move for SPI confirming the details and terms of the move as per agreement reached by phone.
• Every load must be confirmed in writing with a computer generated SPT numbered, load confirmation.
• Please sign the confirmation prior to loading as an indication of your receipt and return by fax to the number indicated on the bottom of each confirmation.
